New Inventions and Discoveries

• Technology Disclosure Form (PDF)
  This is the basic form for submitting an invention to UTRF. You should fill out parts 1-7, have your chair and dean sign parts 8 and 9, and then return the form to your campus research office.

Material Transfer Agreements

  This form needs to be completed when you are going to receive material.

UTHSC Use Only:

• Contract Certification
  This certification form needs to be completed when any MTAs, confidentiality agreements, or sponsored research agreements are being submitted to the Office of Research Administration for signature. You should initial as the “Originating Office.”
